Custom made-to-order service dog ID Badge/Card with your dog's information and photo. The ID is created on PVC Plastic card which is similar size and thickness as a credit card as they are flexible and rigid. We do not require any proof or personal medical information from you. We are very flexible on how you want the ID card to show.
This will include the clear vinyl badge holder and strap clip as shown in the picture.

Return Policy
All Sales are final. A Plus ID in its sole discretion, will approve a request for a return authorization (RMA). Returns without an authorization (RMA) will not be accepted. A Plus ID cannot accept returns on Opened Software, Opened Ribbons, Open Accessories, Printers, ID Systems and Custom Orders. All other orders are subject to a 25% restocking/processing fee. Original shipping charges are not refunded on returned items. Customers are responsible for all shipping charges back to A Plus ID on returned items. All products returned MUST be 100% complete, contain All original boxes and packing materials, have original UPC codes on the manufacturer boxes, contain all manuals, blank warranty cards and other accessories and documentation provided by the manufacturer. A Plus ID offers a 30 day return policy on selected items based on manufacturer return policies. For your protection, A Plus ID recommends you fully insure your return shipment in the case it is lost or damaged, and that you use a carrier that can provide you with proof of delivery.
